Cats sometimes do void their bowels or bladders if startled.  It could be that this is happening when he gets a fright, but it may also be if he's constipated, so I'd maybe get him vet checked, just to make sure there's nothing else going on.

Nah, it's not gross - it's just life.  There's always a bit of poo in there somewhere.   ;)
