Title: Authorisation for re-homing cat form

I am sure this has been mentioned before but we are due to pick up a cat in Portsmouth and need to get the owner to sign a form relinquishing ownership to us.  As I normally rescue from Vets etc or take in strays, have never used a form before but this one is a bit of a complicated one so want to make sure I cover all my bases.

Has anyone got a form I could adapt that covers all the legal side of things?

Any help would be most appreciated.  :thanks:

we have one I will email to you - its very basic and not well thought out but does the necessary!
